Forms
Please see below for the forms needed for both the student(s) and parent/guardian volunteers “POD”. These forms are to be completed for children whose enrollment has been confirmed by the Volunteer Membership Chair. All POD volunteers must complete the forms and required fingerprinting clearance.
Students
These forms are required to be completed prior to the start of the school year. Note, several forms need your child’s physician’s signature.
Parent Volunteers
These forms are required to be completed by at least one parent/guardian per family to fulfill the volunteer role of Parent-of-the-Day (POD) for your student’s class.
